I cannot believe for a minute that TOGA was accidentally pressed. The position of the switches is such that it takes a conscious effort even in the worst turbulence. After twenty six years flying the airplane I never once did it nor did I see anyone else do it.
In reality every revenue passenger flight is a combi flight because freight is carried below. Real "combi" flights require combi aircraft and American does not have any. Nor do any U S carriers.
There is a reason Western built aircraft have a better safety record than those from some other regions of the world. How on earth can you have a thrust reverser system that allows thrust to increase without the actual reverser deploying?
Really, nothing unusual happened here. The crews chose to not make an approach during a thunderstorm. They held for awhile then diverted for fuel.No big deal. It happens every day. If you think every airliner is landing with hours of fuel left in the tanks, think again.
