If memory serves, American Airlines had a DC-10 cargo door open over Windsor, Ontario, and lost a casket of human remains which, to my knowledge, has never been found. Turkish Airlines also had a cargo door open on a flight to Paris and unfortunately that resulted in a multiple fatality crash.
Oh well. Somewhere there is an image of a cargo CV-880 landing in (MIA?) with a cargo door wide open. The difference is the 880 was built like a tank, so just close the door and carry on.
Having pieces falling from the airplane is never a good. Mechanically designing a door to function as a plug door requires complex mechanisms and lubrication and keeping up with wear and tear because of hysteresis. I've seen flight attendants who could not close a main cabin door which is about 50% of their job. It will be interesting to follow on with what the investigation reveals.
