AAIB Report: Boeing 737-86N, unstable approach and loss of altitude on go around

After initially climbing away as expected during the go-around, the aircraft then descended for over 30 seconds reaching a minimum radio altitude of 457 ft. Simultaneously, the crew and the controller realised the aircraft was not climbing away as they expected. Sheesh, that was some dangerous button pushing there. It appears that the tower controller and especially his assistant saved the day. Give those two a bonus.

Descending for over 30 seconds on a go around, while hand flying??? Where was the instrument scan? Children of the magenta blindly locked onto a flight director. They say aviation is unforgiving but this crew proved that wrong.

No kidding. If memory serves, if you forget to set the MA altitude in the alerter and switch from G/A mode to LNAV/VNAV the flight director wants to fly down to the lower preset altitude whether it is MDA or Minimums? Checklist.....challenge & response?

Sounds like an error from potentially a low-time cadet pilot fresh from the academy.
