The captain had placed his personal digital SLR camera between the captain’s seat armrest and the left-hand side-stick controller during a flight from the U.K. to Afghanistan. And another reason that Preacher said they were not done right due to the fact they were not interconnected properly and that is why these things happen.

Not as tragic as the C-130J loss in Kandahar where a pilot stuck his NVG case behind the control yoke while the aircraft was on the ground so that the elevators could be fully deflected to ease loading/unloading. He forgot the case was there when they later attempted take-off. The plane pitched up sharply, stalled, crashed, and killed all 14 aboard.

Bank, bank, bank! The only action to counteract an uncommanded pitch up. At some point the nose will fall and your ailerons then become your pitch control until you figure out the problem.

According to the USAF AIB, the co-pilot apparently thought the uncommanded pitch up was a trim problem and this led to "improper recovery techniques". The aircraft impacted the ground just 28 seconds after rotation within the confines of the airfield.

Airplane was in a 12 year, 6 months prior to this. Found half a socket, missing bolt, deformed bolt, sheared bolt. The last two were probably from the force we put on the yoke to break loose...

Shouldn't that have been caught by a pre-flight checklist? Not being a pilot, I'm not sure what all is included on those. Is checking the yoke operation normally on the list? I thought it was.

It should have been caught if they attempted to move the yoke through all its axes of motion. Unfortunately this was at night and the pilots were wearing their NVGs and the case would not have been visible in the dark cockpit and whatever control surface or yoke movement check they carried out, they didn't catch the pulled back position of the yoke which itself may have been virtually invisible in the dark as well.

I always hated side sticks, this is one of the reasons among many MANY other reasons. All having to do with safety and aircraft control.

I agree about the sidestick controllers not being coupled is very problematic and I believe was an issue in the AF447 crash where the pilot, recognizing a stall, had the stick pushed forward and the inexperienced first officer, not recognizing the alpha floor protection system had reverted (and not trained for it), had the stick pulled back, with the aircraft averaging the two inputs!

However, the Military aviation Authority interim report credits the flight envelope protection system for saving the RAF aircraft from a crash.
