I use a combination of an ACARS and Planeplotter. Planeplotter gives you flight number and ICAO hex code of the aircraft but no registration or type, and ACARS gives you the flight number and registration but no hex code or type. Put them both together and voila !
A decent airband radio helps as well. I use the AR8600-Mk2 with a home-made half-wave airband dipole which cost me about £5 to make and which works just as well as a commercial antenna costing 10 times the price.
The important bit is getting everything set up beforehand rather than just running outside on the spur of the moment, doing a point-and-shoot, and ending up with an image that looks like a fuzzy blurred mess.



and I notice that FA has tagged this as a 742 which was not what I uploaded it as. It left my keyboard as a 744. This is becoming a regular occurrence. We can't ALL be guilty of hitting the 2 key instead of the 4 !!
